The Discharge-Alert Quality Improvement Initiative (2010-2012) enrolled 22 US-based hospitals to screen over 12,000 hospitalized patients in order to determine if alerting the physician about the importance of continued Venous Thromboembolism (VTE) prophylaxis just prior to the time of planned hospital discharge would lower the incidence of outpatient VTE. This was an investigator initiated quality improvement initiative project.
